Email Change Form

- PLEASE READ THESE INSTRUCTIONS!

E-MAIL CHANGE REQUEST

- Please fill out the form completely and carefully, or your request will not be processed.
- Use this form to request a manual e-mail change for your username, in cases such as:
a. Your e-mail on record with the username is expired/deleted;
b. Your e-mail on record with the username does not receive mails from our mail server (Confirmation links, New Passwords...etc.);
c. Your e-mail on record with the username has been compromised/stolen: Only if your attempts to recover it by contacting the e-mail provider or use their 

password recovery procedures were unsuccessful.

1) Create a new valid e-mail - We recommend the e-mail from your Internet Service Provider (ISP) or a reliable e-mail address. Make sure your e-mail address 

is functional and secure.

2) Compose an e-mail with the destination cservice@nationchat.org and put as subject: E-mail change request.

3) Simply COPY/PASTE this entire form into the e-mail body, then supply the following information:

b. Old E-mail Address:

Note: If you have forgotten your e-mail on record with the username, follow these instructions:
- Go to http://cservice.nationchat.org/live , then login with your Username and Password;
- In the User Information section, next to Email click on Open Popup;
- A Popup box will appear asking you to supply the answer to the verification question of your username;
- Fill in the verification answer, click on Submit and your e-mail on record with the username will be shown.

(This is the answer to one of the questions below that you selected when you registered the username)

- Please indicate which question you previously selected by writing the answer next to it: 

a) What's your mother's maiden name?
b) What's your dog's (or cat's) name?
c) What's your father's birthdate?

NOTE: If you have forgotten your verification question/answer, provide 2-3 guesses of answers you might have selected.

e. Channels you have access on:

f. Comments (if any):

* Allow 24 hours for a reply to your e-mail change request.

* Join #Usernames if you have any questions about the e-mail change request.
